A significant “Welsh gender surgery pause” has begun following a decision by health officials. The NHS Wales Joint Commissioning Committee (NWJCC) immediately halted new referrals and scheduled procedures. This action took effect last week, causing considerable disruption for many.
Wales’ Health Minister, Mabon ap Gwynfor, has called an urgent meeting regarding the Welsh gender surgery pause. He plans to discuss the immediate suspension with NWJCC members on Friday. The health board overseeing the Welsh Gender Service, Cardiff and Vale University Health Board, received instructions for the halt.
Concerns over referral rates prompted the decision. Data for 2025-26 indicated “significantly higher rates of referral for gender affirming surgery” in Wales. These figures exceeded those seen in English gender services, according to the health board.
The NWJCC described its move as “precautionary and clinically-led.” Consequently, it has requested an independent clinical review. This review will examine the situation thoroughly.
The suspension directly impacted some patients already awaiting procedures. On Tuesday, Cardiff and Vale University Health Board confirmed patients were in hospital when informed of the halt. These procedures are irreversible, covering both masculinising and feminising surgeries, often involving genital reshaping.
Meanwhile, the decision has drawn mixed opinions. Reform UK and Welsh Conservatives welcomed the pause. Welsh Reform leader Dan Thomas noted “sensible-minded people” would approve. He also urged for a comprehensive service review. Similarly, Conservative health spokesperson Natasha Asghar called the cancellation “absolutely the right thing to do.”
However, many have voiced profound disappointment. A letter, signed by 26 Plaid Cymru Senedd members, expressed their strong concerns. Sent to NWJCC chairman Ian Green, it highlighted the “distress, fear and a deep sense of uncertainty” among the trans community. This situation arises at a time when trans rights face increasing pressure across the UK.
The Plaid Cymru group asked for specific reasons behind the referral concerns. They also sought clarity on when surgeries and referrals might resume. Therefore, pressure is mounting for clear answers.
Health Minister Mabon ap Gwynfor respects the clinical basis of the decision. Nevertheless, he has pressing questions for the NWJCC. He wants details on the review’s duration and format. Moreover, he aims to reassure the public that vital NHS services remain dependable.
First Minister Rhun ap Iorwerth also acknowledged the committee’s decision. He believes it stemmed from “the right reason” concerning service quality. However, the Plaid leader emphasized his desire for a “resolution quickly.” He called for the review into surgery services to be swift.
The Welsh Gender Service refers patients living in Wales. All actual NHS surgeries, however, take place in England. Therefore, the pause affects a critical cross-border service provision.
